Form submissions ​
Form submissions shows all completed forms within your organization. From the overview you create new submissions based on a form template, filter by period, form or creator, and open a submission to view its answers, transcriptions and links.

Open the overview via Forms in the navigation. For each submission the list shows the form, the linked client, the linked appointment, who created the submission and the date. Submissions belonging to a cancelled appointment are marked Cancelled. Click a row to open the submission.
Filter by date, form or creator ​
- Click Filters in the top right. The filter panel slides open.
- Set one or more filters:

| Created by | Limit the list to submissions by a single user. |
| Hide forms of cancelled appointments | Hides submissions linked to a cancelled appointment. |
The number of active filters appears as a badge on the Filters button. The chosen filters are kept in the URL, so you can share or reopen the filtered list.
Apply or clear filters ​
- Click Apply at the bottom of the filter panel to refresh the list with the configured filters.
- Click Clear to remove all filters and show all submissions again.
If a filter combination returns no results, an empty state appears with a Clear filters button.
Create a submission ​
- Click New form in the top right.
- In the Select a form dialog, search for and select the form template you want. Only active forms are shown.
- Click the template to create the submission.
Scrivio opens the new submission straight away so you can start filling it in.
View answers ​
In the overview, click a submission to open the detail view. At the top you see the form title, the creator, the linked client (if any) and the creation date.
The main area lists the questions with their answers. Changes to answers are saved automatically.
Delete a submission ​
- Open the submission.
- Click Delete in the top right.
- Confirm in the Delete form? dialog with Delete.
Note: this action cannot be undone. Associated transcriptions are deleted as well.
After deleting, you return to the overview.
Link a client ​
In the detail view a Linking panel appears on the right (available with the full license tier).
- In the panel, click the Client field and type to search.
- Pick the right client from the list. The link is saved automatically.
- Click the cross next to the linked client to remove the link.
Link an appointment ​
- In the Linking panel, click the Appointment field and type to search.
- Pick the right appointment from the list. The link is saved automatically.
- Click the cross next to the linked appointment to remove the link.
When a submission is linked to an appointment, that appointment's transcriptions are shown in the submission.
View transcriptions ​
The transcription panel is at the top of the detail view. It shows the transcriptions that belong to the submission:
- If the submission is linked to an appointment, that appointment's transcriptions are shown.
- Otherwise, the transcriptions linked directly to the submission are shown.
When you change the appointment link, the transcription panel refreshes automatically.
